    This file include the following items:

  • identification and familial datas
  • a genealogical tree
  • the history of the disease
  • data from each follow up consultation with
  • graphical output for height and weight (growth curve with SD),HbA1c,BP,microalbuminuria
  • evolution of diabetes (severe hypos,ketosis,readmissions into hospital)
  • certificates and reports attached to the file
